Non-surgical endodontic therapy may include-as a complementary step-a surgical sequence.
Persistent periapical pathology-as primary endodontic treatment failure-requires a surgical stage-as conservative therapy.

Secondary endodontic therapy (endodontic re-treatment) could be a non-surgical or surgical tratment.
Both types of interventions are conservative and they are alternatives to tooth extraction.

As non-surgical alternative, endodontic surgery is–at the moment-a minimally interventional, minimally invasive technique, different from traditional techniques which are using dedicated tools, preserving anatomical dental and periodontal structures.
Functional and esthetic appearance are properly maintained, endodontic surgery having such a high rate of success.
