Corticotomy is a procedure commonly used in surgically assisted orthodontic treatment, particularly for adult patients. It involves creating a flap to detach the gum from the underlying bone and performing selective alveolar decortication, which includes making precise incisions in the bone between and above the roots of the teeth. These incisions are limited in both surface area and depth. Afterward, the gum is repositioned with sutures.
The aim of this surgical procedure is to induce a sterile inflammatory response within the bone, leading to temporary demineralization (osteopenia). This, in turn, facilitates orthodontic tooth movement, which can be up to four times faster than conventional treatment.
What are the benefits? The primary benefit of corticotomy is a significant reduction in the total duration of orthodontic treatment, particularly advantageous for patients with compromised bone and/or gum support. Additionally, it helps minimize the associated risks of prolonged orthodontic treatment.
Depending on the individual case characteristics, the surgical intervention may also involve bone and membrane grafts to augment areas with thin or absent bone, as well as soft tissue grafts (gum) in areas at high risk of developing gingival recessions.
